Vision Insurance
The City of Fontana offers optional coverage through Anthem Blue View Vision. The Policy provides full coverage for Covered Services, less a $10 copay for the examination, when you go to a Participating Provider of Anthem Blue Vision. While benefits and savings are typically greater from in-network providers, members are free to visit an out-of-network provider. You have the option of receiving an eye exam from one provider and filling your prescription at another location.

Additional Life Insurance and Accidental Death & Dismemberment (AD&D)
- Additional Life insurance can be purchased in multiples of $10,000 to a maximum of $500,000 or 5 times annual salary, whichever is less
- The cost for additional life insurance is based on an employee's age
- A Guaranteed Issue coverage amount of $110,000 (employees only) may be purchased during the employee’s initial benefits enrollment or during open enrollment
- An EOI form is required when purchasing Additional Life insurance above the guaranteed issue coverage amount
- The EOI is subject to approval by the underwriting company and may require a medical exam
Optional Spouse Life/AD&D
- An employee may apply for Optional Spouse Life insurance in multiples of $5,000 to a maximum of $300,000
- The cost for Optional Spouse insurance is based on the spouse's age
- A Guaranteed Issue coverage amount of $25,000 may be purchased during the employee’s initial benefits enrollment or during open enrollment
- An EOI form is required when purchasing additional life insurance above the guaranteed issue coverage amount
- The EOI is subject to approval by the underwriting company and may require a medical exam
Optional Dependent (Children) Coverage
Optional Life insurance may be purchased for dependent child(ren) as long as the employee is enrolled in additional life insurance coverage. Children are eligible from 1 year to 26 years of age.
- $0.95 per month for $5,000 death benefit (for one or more dependents)
- $1.90 per month for $10,000 death benefit (for one or more dependents)
Age Calculation
- Age for life insurance is calculated once a year at open enrollment and is based on the age which the employee will turn during the coverage year despite the month at which this occurs. For example, if an employee will turn 40 in October during the following coverage year, the employee’s monthly rate will be based off of age 40 for all 12 months. Spouse ages are subject to the same annual calculation.

The City of Fontana is helping you reduce your taxes and increase your take home income by offering a health care flexible spending account (FSA) which allows you to pay for qualified out-of-pocket medical expenses on a pre-tax basis.
Contribution Limit
The maximum contribution for a Health Care FSA is $3,050 for 2023.
Use-It-Or-Lose-It Rule
- The “use-it-or-lose-it” rule is a provision in the IRS Regulations. Under this provision, all money contributed to a FSA must be used to reimburse qualified expenses incurred during that plan year
- Money not used to reimburse eligible expenses is forfeited. The unused portion of your health care FSA may not be paid to you in cash or other benefits including transferring money between FSAs
- To reduce risk of forfeiture, it is critical for you to be conservative when choosing your annual election amount

A dependent care flexible spending account (FSA) allows you to pay for employment-related dependent care services related to care of a qualifying individual on a pre-tax basis.
Contribution Limits
- Your election may not exceed the maximum amount specified in Section 129 of the Internal Revenue Code. Currently, the maximum contribution is $5,000 per year, ($2,500 if you are married and file separate returns)
- Your maximum allocation may not exceed the earned income limitation
- If you are single, the earned income limitation is your salary (excluding your contributions to the plan)
- If you are married, the earned income limitation is the lesser of your salary (excluding your contributions to the plan) or your spouse’s salary
Qualifying Individuals
- A dependent under the age of 13 who resides with you and for whom you are entitled to a personal tax exemption as a dependent (as defined by Code Section 151 (C); however, if you are divorced, the child is a qualifying individual with respect to you if the child lives with you even if you have permitted the non-custodial parent to take the exemption
- A spouse or other tax dependent that resides with you and is physically or mentally incapable or self-care.
