Medical Plan Information
Health insurance is one of the most critical benefits offered by the City. A major illness or injury could be financially devastating without adequate insurance. With this in mind, our benefit program is designed exclusively to meet the health care needs of you and your family. The City of Fontana currently offers the following medical plans:

Group Life Insurance
The City of Fontana provides all full-time and classified part-time employees with life insurance through The Hartford. Our Group Life and Accidental Death & Dismemberment (AD&D) insurance offers financial protection by providing you coverage in case of an untimely death or an accident that destroys your income-earning ability. Employees are entitled to coverage as dictated by their bargaining group.

Benefit Reduction Schedule for Group Life Insurance
After you reach age 65, you will still have group life insurance benefits available to you, however, they will be reduced to 35% of the original coverage amount. Upon turning 70 years of age, your group life insurance benefits will reduce to 50% of the original coverage amount. All benefits will terminate at the employee’s retirement or termination of employment.
Beneficiaries
Employees should review their life insurance beneficiaries after any major life event (birth, death, marriage, divorce, etc.) to ensure that the appropriate individuals will receive their life insurance benefits in the event of an untimely death. Employees can update life insurance beneficiary designations at any time by visiting the Human Resources office.

Disability Insurance
The City of Fontana provides employee's with short-term disability (STD) and long-term disability (LTD) benefits in the event of a non-work related illness or injury that requires employee's to be off work for more than 30 days. All full-time and classified part-time employees are required to hold disability insurance. An employee's disability provider is determined by their bargaining unit as indicated below.

Elimination Period & Benefit Duration
The Elimination Period refers to the length of time of continuous disability which must be satisfied before you are eligible to receive benefits. For short term disability, the elimination period is 30 days and benefits can be received for up to 9 weeks. For long term disability, the elimination period is 90 days and benefits can be received for the entire period during which you continue to meet the definition of disability or age 65.
PORAC
Members receiving long-term disability benefits from PORAC may be eligible to waive their elimination waiting period if the disability is considered industrial. Benefit duration varies upon the reason for the disability. For each continuous period of disability caused or contributed to by a mental disorder, benefits are limited to 12 months or as long as the employee is hospitalized. For each musculoskeletal and connective tissue disorder period of disability, benefits are limited to 23 months. For each disability period resulting from drug and/or alcohol use benefits are limited to 12 months in a lifetime.
Employees can file a claim online by registering for an online account via unum.com/claims or by downloading the MyUnum for Members app.

- Upload documents - medical records, claim forms, authorization forms, etc by using a smartphone camera
- View status updates, payment info, and requests for additional information on your open claims
- View recent letters and documents online
- Go paperless and opt in to receiving text message and alerts virtually

Benefits are paid out in either weekly (STD) or monthly (LTD) and are based off of your earnings prior to becoming disabled.
- Maximum weekly or monthly benefits are equal to 66.67% of your pre-disability earnings to a maximum benefit of $1,615 per week/$7,000 per month ($10,500 for Executives).
Continuing Benefits into Retirement
Employees who were carrying benefits upon retirement are eligible to continue those benefits after they retire at their own cost. The City of Fontana offers employees the benefit of purchasing insurance coverage at the reduced group rates. Since employees will no longer be receiving monthly contribution amounts, the entire cost of the coverage elected will be billed to the employee.
