In the event an incident occurs, the affected person/group/business may file a claim with the City of Fontana.
A claim form (PDF) must be submitted to the City Clerk’s Department (PDF) within six (6) months of the date the incident occurred. If there is not enough space, please use additional paper and identify information by paragraph number.
