Homeless Solution Services

Our Mission: 

 Our purpose is to resolve homelessness in Fontana by uniting the community around sustainable and dignified solutions. Through collaboration with local organizations, faith-based groups, and community leaders, we strive to empower individuals to regain their independence, restore their self-worth, and build a brighter future. 

 If you are experiencing homelessness or are at risk, please contact your case manager for assistance with housing and local resources. 

 If you do not have a case manager and experiencing homelessness in the City of Fontana, you can request one by completing the case management request form linked below. This application form serves as a referral service. A staff member will follow up with you during normal business hours after your request is submitted.
